A parent or guardian must complete an Application for Enrolment form for consideration by the Principal. When completing enrolment application, the following information will be required:

  • completed application for enrolment form
  • original copy  of birth certificate
  • original copy of Proof of Address – examples include Rental Agreement, Shire Rates, Bank Statement, Utilities Statement
  • original copy of Immunisation Record (ACIR Immunisation history statement).
  • Instructions for ACIR

Enrolment Procedures

Once your application for enrolment has been accepted the following information will be required.

  • A completed enrolment form
  • Details of disability or medical condition (if applicable)
  • Family court orders (if applicable)
