A parent or guardian must complete an Application for Enrolment form for consideration by the Principal. When completing enrolment application, the following information will be required:
- completed application for enrolment form
- original copy of birth certificate
- original copy of Proof of Address – examples include Rental Agreement, Shire Rates, Bank Statement, Utilities Statement
- original copy of Immunisation Record (ACIR Immunisation history statement).
- Instructions for ACIR
Once your application for enrolment has been accepted the following information will be required.
- A completed enrolment form
- Details of disability or medical condition (if applicable)
- Family court orders (if applicable)
